Medicare Vision Benefits: What's Covered and What's Not

Age-related eye conditions like macular degeneration, glaucoma, cataracts, and diabetic retinopathy become more common after 65. Yet many seniors are surprised to learn that Original Medicare provides very limited vision coverage. Here's what you need to know.
Vision Services Covered by Original Medicare
Original Medicare (Part B) covers a handful of vision-related services, but only when they're medically necessary — not for routine care.
The Cataract Exception
After cataract surgery with an intraocular lens implant, Medicare Part B covers one pair of prescription eyeglasses (frames + lenses) or one set of contact lenses. This is the only situation where Original Medicare pays for corrective lenses.
Getting Comprehensive Vision Coverage
Medicare Advantage Vision Benefits
Most Medicare Advantage plans include vision benefits that go beyond what Original Medicare offers. In 2026, approximately 94% of MA plans include routine vision coverage. Common benefits include:
- Annual routine eye exam — typically $0–$25 copay
- Eyeglasses allowance — $100–$300+ per year toward frames and lenses
- Contact lens allowance — typically same as glasses benefit
- Access to vision retail networks (LensCrafters, Pearle Vision, Visionworks, etc.)
Standalone Vision Insurance
If you prefer Original Medicare, you can purchase standalone vision insurance. Plans typically cost $10–$25 per month and cover annual eye exams and provide an allowance for eyewear. VSP and EyeMed are two of the largest vision plan networks.
Common Eye Conditions Medicare Does Cover
While routine care isn't covered, Medicare Part B does cover the diagnosis and treatment of eye diseases and conditions. If you're experiencing vision changes, don't delay seeking care — these diagnostic services are covered.
- Age-related macular degeneration (AMD) — including anti-VEGF injections
- Glaucoma — screening, diagnosis, and treatment including eye drops
- Diabetic retinopathy — annual screening and treatment

- Dry eye syndrome — when medically diagnosed and treated
